DPC(NY) • Chazen Engineering Consultants, LLC(TN/OR) Louden Road Residential Conservation Subdivision—PB19.004—15t Submittal Review February 7,2020 Page 2 of 4 Note that the New Water Service Connection Agreement and Application Form, Water and Sanitary Sewer Engineering Report and a cost estimate were not received with the subdivision application. Following are our observations/comments: General: 1. The City's project number of 20191216 should be added to all site plan application, site plans and all reports/documents submitted. 2. The property is located within both the City and Town of Wilton, which requires approvals from both municipalities. Please provide copies of the resolutions from the Town approving the portion of the project within their jurisdiction. Full Environmental Assessment Form (FEAF): 3. Please respond to: D.2.d.iv. 4. The applicant identified that the project is located over a principal aquifer. Please indicate the name of the principal aquifer in the space provided. Subdivision Plans 5. The boundary and topographic survey shall be signed and sealed by a licensed land surveyor. Additionally, any existing established benchmarks shall be indicated on the survey map. 6. The Subdivision Plat must be signed/sealed by the licensed land surveyor. 7. Please label on the Subdivision Plat the minimum permeable area required as well as the maximum impermeable area allowed for each lot. 8. The name of the property owner should be added to the title block of the drawings. 9. In accordance with City subdivision requirements for Conservation Subdivisions, preserved open space land shall be clearly delineated and labeled on the subdivision plat as to its use, ownership, management, method or preservation, and the rights, if any, of the owners of lots in the subdivision and the public to the open space land. The plat shall clearly show that the open space land is permanently preserved by a conservation easement and include deed recording information in the County Clerk's office for the conservation easement. 10. The deed restricted area shown on the subdivision plat should not include the driveways to each home. The Subdivision Plat should be updated to reflect this and quantify the total area of deed- restricted open space on each lot. 11. In accordance with City subdivision requirements (City Code Chapter 241, Appendix H —General Requirements and Subdivision Design Standards, Section 2-R), all applicants for proposed subdivisions are to offer the City 10%Class A Type Usable Land, or an acceptable alternative defined in the City Louden Road Residential Conservation Subdivision—PB19.004—15t Submittal Review February 7,2020 Page 3 of 4 Code. The plans do not indicate any land that has been offered to the City. Please identify how the code requirement will be met. 12. Proposed grading shall be further refined to depict how surface runoff will be directed around proposed homes and absorption fields. Additional spot grades may be necessary to clarify this. 13. Silt fence shown on the erosion and sediment control plans are not shown parallel with contours as required by New York State Standards and Specifications for Erosion and Sediment Control. Please revise accordingly. 14. Insufficient information and detail are provided for the proposed septic tank and onsite disposal system to verify compliance with NYSDOH requirements presented in "Residential Onsite Wastewater Treatment Systems Design Handbook". Specifically, please show the following: a. NYSDOH regulations state 2 percolation tests must be performed within the limits of each proposed absorption field. The only lot that has a percolation test within the absorption field is lot 5, and there is only 1. Additional testing is required in accordance with NYSDOH requirements. b. The proposed septic system layout, showing the laterals, expansion area, septic tank and distribution box locations for each lot. c. Proposed invert elevations of all system components along with top of concrete and the finished grades over each component. d. Certification and tightness testing requirements. e. It appears the areas of the septic fields shown on the site plans are not consistent with the basis of design presented for 4-bedroom homes on sheet C9 which will likely result in a greater area of disturbance. Please revise accordingly. 15. Please show the location of the water service connections and shutoff valves from each of the proposed wells to the house on the Development Plan and indicate size and material of the service. 16. Note, prior to the issuance of a building permit,two well yield tests shall be conducted twenty-four (24) hours apart with one hundred percent (100%) recovery required between tests. Results shall meet the requirements of the Water Quality Standards for Individual Wells, in accordance with the City subdivision regulations and NYSDOH requirements. 17. Please indicate the locations of existing and proposed utilities (gas, electric, cable, etc.) on the Development Plan. 18. It appears that some of the driveway culverts may not have the required cover based on the proposed inverts. Please verify. 19. Please provide a construction detail indicating material specifications and thicknesses of the pavement sections for the proposed driveways.
