ENTITLED "EXISTING CONDITIONS CRESCENT STREET SUBDIVISION BELLA HOME BUILDERS", DATED 01-15-19. 2. ALL EROSION CONTROL MEASURES SHALL BE INSTALLED AND MAINTAINED IN ACCORDANCE WITH THE NEW YORK STATE STANDARDS AND SPECIFICATIONS FOR URBAN EROSION AND SEDIMENT CONTROL. 3. ALL DISTURBED AREAS TO RECEIVE TOPSOIL, SEED, FERTILIZER AND MULCH TO ESTABLISH A PERMANENT STAND OF GRASS. 4. ALL KNOWN UTILITIES THAT EXIST ON OR ADJACENT TO THE PROJECT SITE HAVE BEEN SHOWN ON THE PLANS. 7. THE CONTRACTOR SHALL TAKE ALL NECESSARY PRECAUTIONS TO PROTECT AND PRESERVE EXISTING UTILITIES. 8. ALL UTILITIES DAMAGED OR DISTURBED BY THE WORK OF THIS CONTRACT SHALL BE REPLACED IN KIND BY THE CONTRACTOR. 9. CONTRACTOR SHALL OBTAIN THE REQUIRED PERMITS FOR WORK WITHIN PUBLIC RIGHTS -OF -WAY AS REQUIRED BY THE MUNICIPALITY AND ALL PERMITS REQUIRED FOR UTILITY WORK ON -SITE FROM THE CITY. 10. CONTRACTOR SHALL OBTAIN ALL PERMITS REQUIRED FOR WORK ON -SITE PRIOR TO COMMENCEMENT OF CONSTRUCTION. 11. THERE SHALL BE NO CHANGES ON THESE PLANS IN ADVANCE OF, OR DURING CONSTRUCTION WITHOUT PRIOR APPROVAL OF THE DESIGN ENGINEER, THE OWNER AND THE MUNICIPALITY. 12. ALL CONSTRUCTION SHALL CONFORM TO GENERALLY ACCEPTED CONSTRUCTION STANDARDS OR AS ORDERED BY ENGINEER. 13. THE CONTRACTOR SHALL COMPLY WITH CONSTRUCTION INSPECTION REQUIREMENTS OF ALL AGENCIES, AND PHASE WORK ACCORDINGLY. 14. THE CONTRACTOR SHALL VERIFY ALL DIMENSIONS AND REPORT ANY DISCREPANCIES TO THE ENGINEER PRIOR TO THE START OF CONSTRUCTION. 15. FIELD ADJUSTMENT MUST BE REVIEWED BY A REPRESENTATIVE OF LANSING ENGINEERING PRIOR TO INSTALLATION. 16. PROTECT NEWLY GRADED WORK AREAS FROM TRAFFIC AND EROSION, AND KEEP THEM FREE FROM TRASH AND DEBRIS UNTIL PHYSICAL COMPLETION OF WORK. 17. CONTRACTORS OPERATIONS ON SITE WHICH SHALL INCLUDE BUT NOT BE LIMITED TO DUST CONTROL, MATERIAL HAULING, FIRE PROTECTION, EROSION CONTROL, ETC. SHALL BE CONDUCTED IN ACCORDANCE WITH CITY OF SARATOGA SPRINGS REQUIREMENTS. 18. LOCATE EXISTING UNDERGROUND UTILITIES IN AREAS OF WORK; IF UTILITIES ARE TO REMAIN IN PLACE, PROVIDE ADEQUATE MEANS OF SUPPORT AND PROTECTION DURING EARTHWORK OPERATIONS. 19. THE CONTRACTOR SHALL COORDINATE MAINTENANCE AND PROTECTION OF TRAFFIC WITH THE CITY OF SARATOGA SPRINGS AND SARATOGA COUNTY DEPARTMENT OF PUBLIC WORKS. ALL MAINTENANCE AND PROTECTION OF TRAFFIC SHALL BE IN ACCORDANCE WITH LOCAL, STATE AND FEDERAL REGULATIONS. ALL NECESSARY SIGNAGE SHALL BE IN ACCORDANCE WITH THE NEW YORK STATE DEPARTMENT OF TRANSPORTATION'S MANUAL OF UNIFORM TRAFFIC CONTROL DEVICES. 20.THE CONTRACTOR SHALL BE RESPONSIBLE FOR ANY TEMPORARY WORK EASEMENTS THAT MAY BE REQUIRED FOR EXCAVATION AND ASSOCIATED DISTURBANCE OUTSIDE OF THE RIGHT-OF-WAY ON ADJACENT PRIVATE LANDS. 21. ALL WATERMAIN AND WATER SERVICE MATERIALS SHALL CONFORM TO THE CITY OF SARATOGA SPRINGS STANDARDS AND APPLICABLE AWWA STANDARDS. 22. WATERMAINS, WATER SERVICES AND APPURTENANCES SHALL BE DISINFECTED IN ACCORDANCE WITH AWWA STANDARDS FOR DISINFECTING WATERMAINS, C-651-14, WITH THE EXCEPTION OF SECTION 5.1, TABLET METHOD. FINAL FLUSHING OF THE HEAVILY CHLORINATED WATER GENERATED FROM THE WATERMAIN DISINFECTION PROCESS SHALL BE NEUTRALIZED AND DISPOSED OF IN ACCORDANCE WITH AWWA STANDARDS FOR DISINFECTING WATERMAINS, C-651-14. 23. WATERMAINS SHALL BE PRESSURE AND LEAKAGE TESTED IN ACCORDANCE WITH SECTION 4 OF THE AWWA C-600 LATEST EDITION. 24. THE CONTRACTOR SHALL BE RESPONSIBLE FOR AND COORDINATE ANY REQUIRED BRACING OR RELOCATION OF ANY UTILITY POLE OR STRUCTURE WITH THE APPROPRIATE UTILITY COMPANY. 25. ALL FILL TO ACHIEVE THE PROPOSED ELEVATIONS SHALL BE COMPACTED TO 95% STANDARD PROCTOR. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE COST AND COORDINATION OF THE TESTING AND DOCUMENTATION OF THE FILL MATERIAL AND THE COMPACTION OF THE FILL MATERIAL. 26. THE CONTRACTOR SHALL TAKE NECESSARY PRECAUTIONS TO PROTECT THE SURFACE OF ALL EXISTING ROADWAYS. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL SURFACE RESTORATION AND REPAIR RESULTING FROM THE CONTRACTORS ACTIVITIES. TREE TRIMMING SHALL BE PERFORMED BY THE CONTRACTOR AS NECESSARY WITHIN THE RIGHT-OF-WAY OR UTILITY EASEMENTS. ALL DISTURBED UTILITIES, DRIVEWAY CULVERTS, LAWNS, MAILBOXES, FENCES, SIGNS, DRIVEWAYS, DITCHES ETC. SHALL BE RESTORED TO THEIR ORIGINAL OR BETTER CONDITION, LINES, GRADES AND POSITIONS. 27. ALL DISTURBANCE LIMITS AND SETBACKS SHALL BE STAKED OR FLAGGED IN THE FIELD PRIOR TO BEGINNING GRADING AND CLEARING ACTIVITIES. 28. THE CITY OF SARATOGA SPRINGS SHALL BE RESPONSIBLE FOR THE OWNERSHIP AND MAINTENANCE OF THE SANITARY SEWER LINES. 29. THE CITY OF SARATOGA SPRINGS SHALL BE RESPONSIBLE FOR THE OWNERSHIP AND MAINTENANCE OF THE WATER LINES. 30. WATER SERVICES AND SEWER LATERALS SHALL BE SEPARATED BY 10 FEET MINIMUM HORIZONTALLY AND 18" MINIMUM VERTICALLY. 31. PRIOR TO CONSTRUCTION: THE LOCATION OF ALL EXISTING UTILITY POLES, OVERHEAD UTILITIES AND UNDERGROUND UTILITIES ON THE SITE SHALL BE DETERMINED BY THE CONTRACTOR. ANY DISCREPANCIES SHALL BE BROUGHT TO THE IMMEDIATE ATTENTION OF THE DESIGN ENGINEER. THE COORDINATION AND COST FOR THE RELOCATION OR MODIFICATION OF ANY UTILITIES SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR AND THE COSTS FOR THIS UTILITY WORK SHALL BE PROVIDED TO THE OWNER AND DESIGN ENGINEER FOR REVIEW. CITY OF SARATOGA SPRINGS PLAN NOTES: 1.) ALL WORK MUST CONFORM TO ALL FEDERAL, STATE AND CITY CODES, SPECIFICATIONS, ORDINANCES, RULES AND REGULATIONS. 2.) THE ELEVATION BASE FOR CONTOURS AND BENCHMARKS ARE TO BE BASED ON THE NATIONAL GEODETIC VERTICAL DATUM, 1929. 3.) ALL REFUSE, DEBRIS AND MISCELLANEOUS ITEMS TO BE REMOVED SHALL BE LEGALLY DISPOSED OF OFF SITE BY THE CONTRACTOR TO A LOCATION APPROVED BY THE CITY ENGINEER. 4.) THE CONTRACTOR MUST SET UP A PRE -CONSTRUCTION MEETING WITH THE CITY ENGINEER PRIOR TO ANY CONSTRUCTION. CONSTRUCTION INSPECTIONS BY A DESIGNATED REPRESENTATIVE OF THE APPLICANT ARE REQUIRED. THE COST OF THE CONSTRUCTION INSPECTIONS IS THE RESPONSIBILITY OF THE APPLICANT/DEVELOPER. 5.) CONTACT THE BUILDING INSPECTOR FOR A BLASTING PERMIT, IF BLASTING IS REQUIRED. 6.) THE CONTRACTOR MUST OBTAIN A STREET OPENING PERMIT FROM THE DEPARTMENT OF PUBLIC WORKS FOR ANY WORK WITHIN THE STREET OR RIGHT OF WAY OF ANY CITY STREET, ROAD OR ALLEY. 7.) ALL POINTS OF CONSTRUCTION INGRESS OR EGRESS SHALL BE MAINTAINED TO PREVENT TRACKING OR FLOWING OF SEDIMENT OR DEBRIS ONTO A PUBLIC RIGHT OF WAY. 8.) NO CERTIFICATE OF OCCUPANCY WILL BE ISSUED UNTIL ALL SITE WORK HAS BEEN COMPLETED IN ACCORDANCE WITH THE APPROVED PLANS; AND AN "AS -BUILT" DRAWING HAS BEEN PREPARED IN ACCORDANCE WITH THE REQUIREMENTS OF THE CITY ENGINEER. 9.) THE APPLICANT MUST VERIFY THAT THE PROPOSED PROJECT CAN ACCOMODATE THE TURNING MOVEMENTS OF ANY FIRE TRUCK THAT THE FIRE DEPARTMENT DESIGNATES. NYSDOH REALTY SUBDIVISION CONDITIONS OF APPROVAL A. THAT THE PROPOSED FACILITIES FOR WATER SUPPLY AND SEWAGE COLLECTION ARE INSTALLED IN CONFORMITY WITH SAID PLANS. B. THAT NO LOT OR REMAINING LAND SHALL BE SUBDIVIDED WITHOUT PLANS FOR SUCH RESUBDIVISION BEING SUBMITTED TO AND APPROVED BY THE NEW YORK STATE DEPARTMENT OF HEALTH. C. E.
