Steve- After our stage 2 review we have the following comments to be addressed: GENERAL COMMENTS 1. Labor Law Section 224 Certification form - All applicants shall complete, sign , and return it with your application. See the attached form 224-F. 2. For all disturbed green areas within NYSDOT R.O.W., add callouts, notes, or a detail with items 610.1403 (topsoil-lawns) and 610.1602 (turf establishment-lawns). 3. Final submission shall be PE stamped construction drawings. 4. Provide truck turn movement plan for southern driveway. 5. Perm 36 required. SHEET C-301 6. Move southern entrance 20’-25 south. Revise layout. 7. One-way exit  Move double R6-2 signs & R5-1 signs near E.O.P. and on each side of driveway.  Add stop bar.  Add arrow marking just east of sidewalk.  Change crosswalk markings to Type S. 8. One-way entrance  Add arrow marking just east of sidewalk.  Change crosswalk markings to Type S. SHEET C-401 9. Verify length of driveway culverts using 1 on 6 side slopes. 10. If possible, increase the driveway culvert slopes. 11. Add callout or note for the infiltration basins storm elevations. SHEET C-501 12. Provide profiles for all utility connections showing Route 67 and all existing utilities crossed. 13. Pits that are within 10’ of E.O.P. will require sheeting and shoring. If sheeting and shoring is flush with the surface, the pits can be covered with a steel plate. If not, temporary concrete will be needed. Add sheeting and shoring details. 2 SHEET C-901 16. Add z-bar reinforcement and breakaway post details for signs. 17. Sidewalk Detail, add Item 608.0101 and Item 304.12. 18. NYSDOT Driveway culverts shall have metal end sections. Add callouts or notes. 19. NYSDOT trench detail shall include item 603.9824, item 203.07, and a minimum trench width of 72”. 21. Driveway detail shall have items 404.0982(1.5” - 9.5 F2 Top Course WMA, 80 Series compaction), 404.1989(2.5” - 19 F9 Binder Course WMA, 304.12 (at least 8” Type II Subbase), 407.0103(Straight Tack Coat), and 418.7603(joint adhesive - for top course vertical asphalt joints) OR Item 4” of 608.020102 can be used for the driveway instead of WMA item numbers. Add detail with item number callouts, dimensions, work limits, and notes. The minimum paving limit shall be 30’ from the outside travel lane or extended to the R.O.W. line, whichever is greater. 22. Concrete Sidewalk Detail shall include items 608.0101 (4”- concrete sidewalk), 304.12 (6”-subbase), 610.1403 (4”-topsoil-lawns) and 610.1602 (turf establishment-lawns). SHEET C-904 23. Replace with Standard sheets 619-010, 619-011, 619-619-311, and 619-414 to plans. Link to New Standard Sheets: https://webapps.dot.ny.gov/work-zone-traffic-control WZTC NOTES TO BE ADDED  THERE SHALL BE NO TEMPORARY LANE CLOSURES ON ROUTE 9 NB, MONDAY-FRIDAY 4PM-6PM.  THERE SHALL BE NO TEMPORARY LANE CLOSURES DURING THE FOLLOWING SPECIAL EVENTS: SARATOGA RACETRACK SEASON OR THE WEEKEND OF THE BELMONT STAKES.  LANE CLOSURES AND/OR USE OF TEMPORARY CONCRETE BARRIER WITHIN FIVE FEET OF AN ACTIVE TRAVEL WAY BETWEEN NOVEMBER 1 & APRIL 15 SHALL REQUIRE SPECIAL PERMISSION FROM THE ENGINEER WITH CONCURRENCE FROM THE REGIONAL TRAFFIC OFFICE. THE CONTRACTOR SHALL NOTIFY THE ENGINEER ONE WEEK IN ADVANCE OF ANY REQUESTED CLOSURE.  PRIOR TO ANY REDUCTION IN ROADWAY WIDTH, THE CONTRACTOR SHALL PROVIDE THE ENGINEER TWENTY-ONE (21) DAYS NOTICE IN WRITING SO HE/SHE MAY CONTACT THE REGIONAL PERMIT ENGINEER OF THE WIDTH RESTRICTION IN A TIMELY MANNER. THE CONTRACTOR SHALL NOTIFY THE ENGINEER IN WRITING IMMEDIATELY UPON THE REMOVAL OF THE LANE WIDTH RESTRICTION SO THE ENGINEER MAY NOTIFY THE REGIONAL PERMIT ENGINEER.  THE CONTRACTOR MUST BE PREPARED TO SWEEP THE ROAD DAILY OR AS ORDERED BY NYSDOT SARATOGA COUNTY RESIDENCY.  THE CONTRACTOR SHALL MAINTAIN PEDESTRIAN AND BICYCLE TRAFFIC THROUGH AND AROUND THE PROJECT FOR THE DURATION OF CONSTRUCTION. MATERIAL, EQUIPMENT OR OTHER BARRIERS SHALL NOT BE PLACED OR MARKED IN SUCH A MANNER AS TO OBSTRUCT PEDESTRIAN OR BICYCLE TRAFFIC OR TO PRESENT A SAFETY HAZARD TO THE NON-MOTORIZED PUBLIC. WHERE PEDESTRIAN TRAFFIC MUST BE RELOCATED OFF THE EXISTING FACILITY, WALKWAYS SHALL BE CLEARLY MARKED AND HAVE A LOGICAL START AND TERMINUS.  HOLIDAY TEMPORARY LANE/SHOULDER RESTRICTIONS TABLE.
