Instructions for Completing Part 1
Part 1 is to be completed by the applicant or project sponsor. Responses become part of the application for approval or funding,
are subject to public review,and may be subject to further verification.
Complete Part 1 based on information currently available. If additional research or investigation would be needed to fully respond to
any item,please answer as thoroughly as possible based on current information;indicate whether missing information does not exist,
or is not reasonably available to the sponsor;and,when possible,generally describe work or studies which would be necessary to
update or fully develop that information.
Applicants/sponsors must complete all items in Sections A&B. In Sections C,D&E,most items contain an initial question that

Brief Description of Proposed Action(include purpose or need):
Demolition of the existing 9,179 square foot one-story concrete block building is proposed as an initial phase of site remediation. The existing building is in
disrepair and has been temporarily stabilized to prevent collapse. Upon demolition of the existing building,environmental remediation will commence,
consisting of removal of soil and in-situ treatment of groundwater(please see attached narrative for full description). This Action seeks City approval of the
demolition of the existing dilapidated building as the structure is located in the Historic Overlay District and/or City approval of demolition of certain
structures requires site plan approval. The remedial action work plan and the environmental review of the remediation is within the purview of the
NYSDEC under the Brownfield Cleanup Program.
This Action does not include the redevelopment of the Property which is the subject of a separate concurrent review. Approval of this Action does not
commit the City or its agencies to approval of the proposed redevelopment. Stated differently,the remediation of the Property is not dependent on the
approval of a redevelopment plan for the Property.
