In accordance with 6 NYCRR Part 617, the Design Review Commission classifies this request as a SEQR:
2 Type 11 action — exempt from further SEQR review
And , in accordance with the objectives, standards and guidelines contained in the City Zoning Ordinance
Article 240-7A Historic Review, the Design Review Commission issues the following decision on

Note : materials to be wood and details and sizes to match existing.
Note : this approval shall expire 18 months from the issuance date unless any necessary building permit
has been issued and actual work begun.
