Site Plan Review Application& Checklist Dated March 2, 2021 B. Environmental Assessment Form Dated February 18, 2021 C. Project Narrative Dated February 2021 D. Lot Line Adjustment Application Dated March 2,2021 E. Site Plans Dated February 2021 F. Stormwater Pollution Prevention Plan(SWPPP)Dated February 2021 G. NYSDEC Endangered Species Letter Dated January 20, 2021 H. OPRHP Letter Dated February 17,2021 I. OPRHP Letter Dated August 4, 2015 J. Engineer's Estimate Opinion of Cost Dated February 26, 2021 As requested we have reviewed these documents and offer the following comments that the applicant should satisfactorily address: General 1. The Site Plan Review application incorrectly identifies the project's zoning district, it should be noted as IND-G Industrial General. 2. The Site Plan Review Submittal Checklist indicates items that are inconsistent with the site plan documents. The documents should be revised to address the following: a) The parcel tax map numbers are not shown except in a table format on Sheet C-101. Both parcels that make up the project should be indicated on the plans; b) The name of the adjacent property owner to the east of the parcel is required to be indicated; c) Plans should clarify the extent of Freedom Way and associated easements that may need to be extended with the Freedom Way extension; d) The site plan approvals obtained for the existing building/site should be noted on plan; Ms.Kari Donohue April 5,2021 Page 2 of 4 e) Details for the dumpster screening should be provided; f) Design/type of construction and materials of buildings should be provided(elevations); and g) The check list indicates that no lighting plan is provided but the landscaping plan shows the proposed lighting. Lighting and landscaping plans should be separate. 3. The project narrative references sheet C-191 for site lighting but sheet C-191 is not provided. 4. The lot line adjustment plat plan should be provided for review. Site Plans 5. Sheet C-000's project location and vicinity maps should be updated to use the site area of the proposed project. 6. Sheet C-101, site statistics should be revised to indicate the zoning district as IND-G. 7. Soil test pit and infiltration testing should be performed at the location of the infiltration basins and dry wells, per Table D-1 of the New York State Stormwater Management Design Manual (the Design Manual). The results should demonstrate that the infiltration basins will function as designed and maintain the minimum required three (3) foot separation between the bottom of the basin and seasonal high ground water. 8. Depending upon the SIC code identified for the project the site may be deemed a hotspot requiring additional stormwater system design to allow for this condition. 9. Plans should include a construction detail for the proposed infiltration basin that is in conformance with the Design Manual and minimally includes: a) Water surface elevations for each design storm; b) The requirement that the bottom of the infiltration basin be located a minimum of 3 feet above seasonal high ground water; c) An observation well,with a detail of its construction and the location also shown in the plan view; d) Construction notes (also provided in the sequence of construction)that indicate the area to be protected from heavy construction equipment traffic and not connected to the storm system until the completion of construction and the final stabilization of all upstream areas;and 10. Infiltration basins are required to provide for the overflow associated with the 10-year design storm event(non-erosive velocities on the down-slope). 11. Concrete Sidewalk Detail(Sheet C-501): a) Expansion joints should be 1/2"wide; and b) The sidewalk cross-slope should be between 1/4" and 1/2". 12. Areas to receive restoration and permanent seeding as noted in the stormwater report should be identified on the plan. Storm Water Management 13. The applicant should: a) Confirm that this project is not required to obtain the NYS DEC SPDES Multi-sector General Permit for Stormwater Discharges Associated with Industrial Activity(Multi-sector Permit)in Ms.Kari Donohue April 5,2021 Page 3 of 4 addition to the NYS DEC SPDES General Permit For Stormwater Discharges From Construction Activity(General Construction Permit); b) Include a statement in the SWPPP stating the project's Standard Industrial Code and a statement as to the need or how the project is exempt from the Multi-Sector Permit requirements. 14. The SWPPP is currently a draft document until authorized by the Town's MS4 designated authority and NYS DEC. The final SWPPP should contain the following documents: a) Trained contractor certifications showing the completion of DEC's Endorsed Four-Hour Erosion and Sediment Control(E&SC )Training; b) Signed copy of the NOI, and the NYSDEC SWPPP Acknowledgement letter of permit coverage; c) Contractor/subcontractor certification statements; and d) Identification of contractor/subcontractor responsible for installation and maintenance of E&SC devices. 15. The SWPPP should include a complete description of potential sources of stormwater pollution, such as: construction debris, litter, chemicals/materials associated with construction, and post construction pollution sources such as petroleum products discharges. 16. A pre-development drainage analysis map should be included. 17. Drainage analysis maps should include the following information: a) Stormwater Analysis points; and b) The time of concentration flow path segments. 18. Per the Design Manual dry wells are: a) Best suited for treatment of rooftop runoff and the tributary area should include rooftop runoff only; b) Required to provide pretreatment to prevent clogging. This should be provided in the form of an underground trap with a permanent pool between the downspout and the dry well; c) Required to have a maximum depth of 4 feet; d) Required post construction stormwater management practice signage; and e) Required to have an observation well. Details of its construction and the location in the plan view should be provided. 19. The detail for Dry Well#1 shows two dry wells,however the plans only show one. 20. There are multiple labels on the Site Plan(Sheet C-101)that are misplaced. Please feel free to call our office if you have any questions.
