Regarding: AT&T Installation on Utility Pole

Dear Owner,
AT&T is looking to install equipment on a utility pole that is owned by National Grid,but
which pole we believe may be located on the boundary line of the utility easements and Best
Western's property. The National Grid pole number is pole 157/1 and it is located near the
southern corner of Parcel ID 191.-1-56.1 in between the New York State Right of Way and the
subject property.
AT&T's equipment will help to increase AT&T's data capacity and transmission speed
within the immediate vicinity of the equipment. This equipment will aid in reaching AT&T's goal
of continuing to provide reliable wireless communications services in and around this area of
Saratoga Springs.
We currently have licenses from National Grid for this installation, however because this
utility pole may not be in the public right of way, the City of Saratoga has asked us to reach out to
you the abutting landowner for permission to install on the utility pole. I have enclosed a copy of
the construction drawings showing AT&T's planned installation.
If the Owner consents to AT&T's installation,kindly sign below in the space provided and

Centerline conducted worst case modeling to determine whether the antenna configuration for
NNV4SSP-360S-F4, is in compliance with FCC Regulations.

measures recommended in this report are implemented.

5.3 DESCRIPTION OF MPE-LIMIT EXCEEDING AREAS
Based on worst-case predictive modeling,there are no modeled exposures on any accessible
walking/working surface related to AT&T's proposed antennas that exceed the FCC's occupational
and/or general population exposure limits at this site.
At the antenna level there is an area that extends 9 feet from the antenna face that may exceed the FCC's
occupational limit for exposure to radio frequency emisssions. There is an area that extends 19 feet from
the antenna face that may exceed the FCC's general population limit for exposure to radio frequency
emissions.
There is an area that extends 4 feet below the bottom tip of the antenna that may exceed the FCC's
occupational limit for exposure to radio frequency emissions. There is an area that extends 5 feet below
the bottom tip of the antenna that may exceed the FCC's general public limit for exposure to radio
frequency emissions.
5.4 CARRIER SIGNIFICANT CONTRIBUTION AREAS
At ground level there are no areas predicted to exceed 1% MPE of the general population limit.

5.5 COLLOCATOR SIGNIFICANT CONTRIBUTION AREAS
Based on review of construction drawings and aerial photographs, no collocators were identified onsite.
Emission exceedances are predicted 9 feet from the antenna face. This antenna must not be installed
within 9 feet of any structures or surfaces that may be walkable.
6.0 FALL ARREST AND PARAPET INFORMATION
As per AT&T barrier policy, rooftop edges that are protected with a 39-inch parapet wall or guardrail are
safe for work activity within six(6)feet of the edge. OSHA has stated that an existing 39-inch guardrail
or parapet provides sufficient protection for employees. The height of the top rail or equivalent
component of guardrail systems in new construction shall be at least 42 inches above the walking or
working surface. It should also be noted that the height of the parapet or guardrail may be reduced to no
less than 30 inches at any point provided the sum of the depth(horizontal distance)of the top edge,and
the height of the top edge (vertical distance from the work surface to the top edge of the top member, is at
least 48 inches. If there is no reason for working atop the roof,then edge protection is not required. In
addition,workers may use personnel lifts or temporary fall protection measures to perform work within 6
feet of the roof edge in place of permanent edge protection. Reference: 29 CFR 1910.28, 29 CFR

APPENDIX B: FCC GUIDELINES AND EMISSIONS THRESHOLD LIMITS
All power density values used in this report were analyzed as a percentage of current Maximum Permissible
Exposure (% MPE) as listed in the FCC OET Bulletin 65 Edition 97-Oland ANSI/IEEE Std C95.1. The
FCC regulates Maximum Permissible Exposure in units of microwatts per square centimeter (µW/cm2).
The number ofµW/cm2 calculated at each sample point is called the power density. The exposure limit for
power density varies depending upon the frequencies being utilized. Wireless Carriers and Paging Services
use different frequency bands each with different exposure limits,therefore it is necessary to report results
and limits in terms of percent MPE rather than power density.
All results were compared to the FCC (Federal Communications Commission) radio frequency exposure
rules, 47 CFR 1.1307(b)(1)— (b)(3), to determine compliance with the Maximum Permissible Exposure
(MPE) limits for General Population/Uncontrolled environments as defined below.
General Population/Uncontrolled exposure limits apply to situations in which the general public may be
exposed or in which persons who are exposed as a consequence of their employment may not be made fully
aware of the potential for exposure or cannot exercise control over their exposure. Therefore, members of
the general public would always be considered under this category when exposure is not employment
related, for example, in the case of a telecommunications tower that exposes persons in a nearby residential
area.
Public exposure to radio frequencies is regulated and enforced in units of microwatts per square centimeter
(µW/cm'-). The general population exposure limit for the 700 and 800 MHz Bands is approximately 467
µW/cm2 and 567 µW/cm2 respectively, and the general population exposure limit for the 1900 MHz PCS
and 2100 MHz AWS bands is 1000 µW/cm2. Because each carrier will be using different frequency bands,
and each frequency band has different exposure limits, it is necessary to report percent of MPE rather than
power density.
Occupational/Controlled exposure limits apply to situations in which persons are exposed as a consequence
of their employment and in which those persons who are exposed have been made fully aware of the
potential for exposure,have been properly trained in RF safety and can exercise control over their exposure.
Occupational/Controlled exposure limits also apply where exposure is of a transient nature as a result of
incidental passage through a location where exposure levels may be above general population/uncontrolled
limits (see below), as long as the exposed person has been made fully aware of the potential for exposure,
have been trained in RF safety and can exercise control over his or her exposure by leaving the area or by
some other appropriate means.The Occupational/Controlled exposure limits all utilized frequency bands is
five(5)times the FCC's General Public/Uncontrolled exposure limit.

Carlson McCain, Inc. (Carlson McCain) is pleased to present this "Utility Pole Structural Analysis Report" to
determine the structural integrity of the antenna mount and equipment on the structure listed above. The purpose
of the analysis is to determine acceptability of the Existing Utility Pole. Based on our analysis we have determined
the stress level for the pole under the following load case to be:
Existing Utility Pole+Proposed Equipment: Sufficient Capacity
Note:See attached report for loading case (Passing at 24.1%)
This analysis has been performed in accordance with the 2015 International Building Code based upon an ultimate
3-second gust wind speed of 115 mph converted to a nominal 3-second gust wind speed of 89 mph per section
1609.3.1 as required for use in the ANSI/TIA-222-G Standard per Exception#5 of Section 1609.1.1.
All the equipment proposed in this report shall be installed in accordance with the construction drawings for the
determined available structural capacity to be effective. See Appendix A for list of proposed equipment.
